Evidence-Based Practice Question

According to research studies in the clinical units, Elder patients suffering from

chronic diseases such as; diabetes, hypertension and stroke, luck a perfect health care unit

or model that could be enhancing their good progress in their health conditions. This lead

them to face the fate of death within a short period time unlike one that they could take if

they had proper medications (Friis). EBP has helped identify this same condition in my

area of work and thus why I have to come up with a question to be researched on i that

"How can elder patients’ health care or treatment process, be managed or monitored in

order to enable them survive with the chronic diseases or even get treated completely and

heal from those diseases?”

Elder patients have a chronic illness and in our professional role, they are not

properly attended and thus why you see rapid increase of elder deaths as evidenced in the

society that we are today. Actually, before I got to understand the importance EBP in my

profession, I had a negative perspective about it, I thought it was just important for

Scientific Researchers, but EBP has helped me come up make a correct decision on how

to handle Elders who are suffering from chronic diseases. Chronic Care Model should be

introduced in medical routines to enhance proper health care activities. Chronic care model

is used to know which kind of improvements the services needs at any particular time. It is

an excellent tool for improving care at both the individual and population level. This model

suggests some approaches like the incorporation of all parts in the services; including the

provider, the patients and the interventions, though it may be seen as expensive and time

consuming hence that being a great barrier to making it as the only decision to make in

order to tackle the Elder patient problem

The Chronic Care Model has various concepts that can improve the health care

system. Examples of the concepts are: the system of the clinical information, the support

inside the organization, the delivery of the system and self-management support. These

concepts are basically focused in the practice and others in the patients (Kubler-Ross).

When it comes to the elder patients in a medical home it refers to a team based

model of care lead by a physician which need to provide continuous and coordinated care

throughout the lifetime of elder patients in order to maximize their health outcomes.

Models like the Chronic Care Model looks for the improvement of the quality and

efficiency of the children and adult primary care and in the healthcare system as stated by

(Skovlund).

Professionals should make decisions based on scientific evidences since, this

technique identifies the root cause of the problem and also it helps in getting the solution

to that problem. They should let the misconceptions they possess like I did on the Evidence-

Based Practices and his will clear all the barriers that are hindering the EBP from being put

into practice.
