Part A (problematic patient/family or staff situation)
Euthanasia ethical dilemma is one of the most challenging to deal with in the nursing
profession. In my last shift, I got myself faced with this kind of dilemma. The patient I was
caring for had sepsis after he had undergone a lumbar stenosis surgery, had C.Difficle,
leukocytosis as well as acute renal failure. All this combined makes the patient sound terminally
ill and hard to heal. However, from my assessment and the care I had provided him with I had
already observed some positive changes in his health and hence there was room for more
improvement. On observing the pain that had the patient was undergoing, and the different
illnesses the patient was suffering from the family had given up and decided to provide palliative
care for the patient.
Part B (personal thought/feelings resolution and learning)
I had committed a significantly adequate time in caring for the patient and I was hopeful
he would recover. On realization that his family wanted palliative care carried out I felt bad
about it. With the knowledge that there was a possibility for the patient to heal I felt hard to let
go the patient on euthanasia. Unfortunately I could not stop the move, overturn the decision or
discourage the family from doing it given that they had the right to do it. This confused me since
I did not want to infringe on the rights of the patient and the family and neither did I want the
patient dead. To overcome all this, I decided to hold a discussion with the family so as to listen

to their thoughts and sell my ideas to them with an aim to come up with a medically reasoned
and informed decision about the patient.
Part C
The family expressed their sincere will and wish to let their old father peacefully die after
learning how critical his condition was. I carefully listened to the rationale or the reasons behind
their resolution and although I didn’t fully like the idea I gave in and supported it. The reasoning
that the family did not want their father to continue suffering and also spend a lot of money on
his medical bill sounded as substantial grounds to me to let go the patient. I learned to respect
family’s’ and patient’s rights on the issue of euthanasia. When faced with the same situation
again I will provide advice to the deciding family first then uphold their decision as I did in this
case.

As a nurse, it feels awkward and is also disturbing to abandon a patient who is in dire
need of care that you can provide. In my last shift, I happened to experience this kind of
scenario. I received a patient who had been involved in a fatal accident whereby he was hit by a
car while trying to cross the 15 freeway. The 31-year-old female patient had her liver physically
injured and was diagnosed of liver laceration grade 111. Additionally, the patient had an open
wound. Unfortunately, the patient was very aggressive and combative as well. She gave no
chance for medication. On touching her, she would scream very loud. Even after leaving the
room and coming back later she would still behave aggressively and yell at the top of her voice
on seeing us.
Part B (personal thought/feelings resolution and learning)
The charge nurse directed my preceptor and me to leave the patient’s room after
observing how difficult it was to provide medication and care to the patient. This troubled me
seeing that I was abandoning a patient who needed urgent attention. If the liver was left
unattended, the health of the patient would deteriorate and possibly lead to other complications.
This is owing to the fact that the liver plays a broad spectrum of roles in the body including
digestive, detoxification and hormonal regulation functions among others. I felt it not fit to watch
the patient undergo more suffering and possible complications thereafter. From the look of the
patient, I concluded that she must have been traumatized something that made her aggressive and
scream at our sight.

I reported the matter to the doctor to see what directions he would give us. The doctor, in
turn, ordered that we should use force to have the patient attended to. When force was applied,
the patient gave in although she was still aggressive. We got a chance to provide medication and
also change the patient’s wet pad. If I encounter such a patient again in future, I will apply the
same method the doctor advised here.
