For a long time, nurses followed only a doctor’s orders. They were categorically forbidden
to make independent decisions regarding the care of patients. However, the development of
nursing and the fight for human rights in the world have become an incentive for change in one of
the main medical professions: the nurses gradually become more independent in her activities.
Different theorists describe the knowledge of nursing according to their research and findings. Jean
Watson and Florence Nightingale are among the leading theorists who researched the
understanding of nursing practice. This study analyzes their views on the knowledge of nursing
within different theoretical patterns.
Florence Nightingale views nursing as a science since it is an environment-related
discipline, while Watson reasons that nursing is a science due to the relationship between caring
and human feelings. For example, Nightingale relates health hazards to unsanitary conditions
whereby the environment components determine disease infection. Noise, ventilation, light,
warmth, and effluvia are the environmental conditions that determine the probability of infection
spread. On the other hand, Jean Watson explains her empirical knowledge through caring and
curing. According to her, caring can be practiced and demonstrated. If caring is of high-quality
and timely, curing would be unnecessary; that is, the recovery stage of a patient will be realized
quickly (Shields, 2014). A caring nurse accepts patients as they are and focuses on what they may
become.
Within esthetic knowledge, Nightingale views the experience of nursing as the driving
forces for human contribution to the healthy environment. On the other hand, Watson regards
caring as a human effort that reduces or increases infection rates. Another difference between the
theoretical views involves personal knowing. For example, Nightingale believes that the

relationship between a nurse and a patient is influenced by reparative powers while Watson views
it as a product of existential-phenomenological force. Considering the ethical knowing,
Nightingale views environmental conservation as a critical step to health maintenance while
Watson upholds respect, care, nurturing, and attaching patients to a high value as the best means
for treatment (Bender & Feldman, 2015). Sociopolitical factors are another area that both theorists
delved into. Nightingale emphasizes that sociopolitical factors determine the condition of the
environment: a protected environment will lead to a healthy society while a poor setting causes
health hazards. Watson reasons that sociopolitical factors determine the type of care administered
in health centers’.
Metaparadigm is a term created by Florence Nightingale during the Crimean War that
refers to the main elements involving nursing profession. There are four metaparadigm concepts
of nursing profession: health, person, nursing, and environment (Vati, 2015). Although the four
elements are different, they have a direct relationship and influence each other. The metaparadigm
concept is based on the health-illness scale at the time of offering nursing services, on the
environment of providing the service, on the patient under treatment and on the interaction of the
nurse and the person under treatment.
The metaparadigm of person is a patient who is under nursing care services and may be
represented by a community, family, and an individual. Nightingale views this element as a patient
who is treated by a nurse, who has a reparative power and who is affected by the environment
(Bender & Feldman, 2015). On the other hand, Watson views a patient as a person who should be
cared for, valued, assisted, understood and respected. As for another example, health concept is
the level of illness, wellness or well-being of a person. According to Nightingale, health is using
person’s power to maintain well-being which is achieved by controlling the environment. In

contrast, Jean Watson views health as a complete social, physical and mental functioning and well-
being.
The third element of metaparadigms is environment. It refers to any place in which the
nursing care is carried out (Shields, 2014). Nightingale explained this component as everything
social, psychological and physical, while Watson related environment with the society. Finally,
the nursing concept of metaparadigm involves all the services that nurses offer to provide care to
their patients. According to Nightingale, nursing as a concept should ensure a proper diet, fresh
air, quiet to facilitate individual’s reparative process and warmth. On the other hand, Watson
considers nursing component as a process aimed at restoring and promoting health besides
preventing illness.
In conclusion, Nightingale and Watson supported a varied knowledge in the nursing
profession. Being the leading theorists of nursing, they provide a detailed concept of the practice.
Their ideas should be applied by nurses jointly to offer quality nursing services to patients. The
above-mentioned theorists have proved to the society that nurses play a determining role in
enhancing the recovery of the patients by applying their skills.
